Gateway now offers online pre-registration.You may pre-register by completing our online registration form at least two business days in advance of the procedure date. You will be given a confirmation number after completing the form. You must also bring your insurance card and a photo ID with you on the day of your appointment.

You may also pre-register by phone by calling 931-221-2741 or 931-221-2729 Monday - Friday 10:30 a.m. to 7 p.m. You must call to pre-register at least two business days in advance of the procedure date.

Please remember that all copayments and unmet deductibles are due at the time of service.
